PBEF is also known as visfatin and was first identified as a factor that enhanced the differentiation of B-cells. It is a secreted factor that is highly expressed in adipocytes. Intracellularly, PBEF acts as a cytosolic enzyme involved in nicotinamide adenine dinucleotide (NAD) synthesis. PBEF levels positively correlate with obesity and type 2 diabetes mellitus and can bind the insulin receptor to lower blood sugar.
